Medical Release Waiver

Uses and procedures:

• The medical release waiver is required to be on-file and current for each student when a trip is taken.
• The medical release waiver must be updated before each trip.
• A new waiver must be filed after every third (3rd) activity.
• Waivers must be signed by a parent or guardian over 21 years of age. After 21 years of age, students may file waivers on their own behalf.
• Waivers must be notarized by a Notary Public with a current commission with the State of Mississippi licensed in Leflore Country.
