Here you will find the paper resources that you will need for your child to attend certain trips and events with our students. The forms will be categorized based upon the trip or event for which they must be used. Note that the NGBC medical release waiver is required for all trips or events unless otherwise noted in the trip/event materials.

Medical Release Waiver

1. North Greenwood Baptist Church Medical Release Waiver (link will open in same window)
   The medical release waiver is a “interactive” PDF. You can open the file using a regular PDF reader, such as Adobe Reader. The difference is that the interactive PDF has blanks that you can fill in by typing in the values on your computer. The medical release waiver is due for all events unless otherwise noted.

Centrifuge

1. Parent Pack (Updated each winter)
   The parent pack, though not required, may be a valuable tool for parents of students going to Centrifuge. The parent pack includes copies of important Fuge documents as well as schedules, email addresses, phone numbers, dress codes, and track times for the camp location we will be going to.

2. Medical Release Wavier (Updated each winter)  **required document**
   The Centrifuge Medical Release Waiver is different from the NGBC waiver and is required for all participants, including chaperones. Please note that TWO signed and notarized copies are needed. You will NOT need a separate NGBC waiver to participate in this activity.
   NOTE: The Fuge Release Waiver is at the bottom of the parent pack PDF.

Spring Break

1. Parent Pack (updated each winter) **CURRENTLY NOT AVAILABLE**
   The parent pack, though not required, could be an important tool for parents with children going on spring break with us. The pack will include various important documents, including: tentative schedules, dress codes, suggested packing lists, etc.

Paintball

1. Paintball Waiver (Under 18)

White Water Rafting

1. Outdoor Adventure Rafting Release of Liability
   Required in addition to NGBC Medical Release Waiver

Scholarships

1. Smith-Edwards Scholarship
   The Smith-Edwards scholarship is awarded each Mother’s Day to a deserving high school, college, or graduate student of North Greenwood Baptist Church. Applications are generally due around mid-April. Please see the main page for updates.
